Sutter VNA & Hospice is one of Northern California's largest not-for-profit, community based home care and hospice agencies, providing high quality health care in patients' homes. Services include visiting nurses, rehabilitation therapists, medical social workers; home infusion therapy; home health aides, hospice care; bereavement programs; and community health clinics. Sutter VNA & Hospice serves nine Northern California counties with local offices in Emeryville, Concord, San Leandro, San Mateo, Santa Rosa, Sacramento,  and Roseville, and manages home health and hospice services in Marin County and San Francisco.

Under the supervision of the Clinical  Supervisor and the AIM Program Coordinator, the AIM Program RN provides specialized nursing services to patients nearing the end of life.  The AIM Program RN manages a caseload of patients with advanced illness and functions as a consultant to other Homecare staff regarding appropriateness of patients for the program, symptom management and end of life issues. The care focuses on symptom management, advanced care planning, education relating to disease process and treatment options, setting goals of care with patient and family, reduction of Emergency Room (ER) visits, inpatient hospitalizations and timely transition to hospice.   Depending on the branch census, the AIM Nurse may also case manage regular homecare patients.  This position works closely with the Hospice staff to ensure high quality symptom management.  This position may also work with hospital palliative care teams and hospitalists to provide seamless care with consistent goals. Other duties associated with the AIM program will be assigned as needed.
